𝗼𝗻 𝗢𝗰𝘁𝗼𝗯𝗲𝗿 𝟭𝟭, 𝟮𝟬𝟮𝟰, 𝗮𝘁 𝟴:𝟬𝟬 𝗮𝗺 𝗮𝘁 𝘁𝗵𝗲 𝗖𝗼𝗹𝗹𝗲𝗴𝗲 𝗔𝘂𝗱𝗶𝘁𝗼𝗿𝗶𝘂𝗺.
Be advised to have your parents with you during the activity. Parents or guardians such as siblings, relatives, and landlady/landlord who will accompany the grantees shall present a valid ID.
Photocopy your school IDs in 4 copies (𝗟𝗢𝗡𝗚-𝗖𝗢𝗟𝗢𝗥𝗘𝗗 𝗕𝗢𝗡𝗗 𝗣𝗔𝗣𝗘𝗥 𝗯𝗮𝗰𝗸-𝘁𝗼-𝗯𝗮𝗰𝗸 𝘄𝗶𝘁𝗵 𝘆𝗼𝘂𝗿 𝘀𝗶𝗴𝗻𝗮𝘁𝘂𝗿𝗲 𝗮𝗻𝗱 𝗱𝗮𝘁𝗲 𝗼𝗻 𝘁𝗵𝗲 𝗿𝗶𝗴𝗵𝘁 𝘀𝗶𝗱𝗲). Ensure that your signature on your school ID matches your signature on the paper.
